On April 25, at approximately 9:16 a.m., the Town of Montgomery Police responded to an industrial accident at L and W Supply located at 155 Bracken Rd. in the Town of Montgomery.

The incident was the result of a forklift striking an individual. This action caused the death of Homer D. Baker 54, of Waycross Georgia.

Baker was a truck driver who worked for Western Express and had just arrived to make a delivery.

The Town of Montgomery Police were assisted by the New York State Police and the Orange County Medical Examiners Office.

The investigation remains ongoing.
